Insurance and Payment information

Payment information

  1. Payment is due at the time of services rendered by Dr Jessica Evelson, MD

  2. All cancelations for scheduled appointments for both psychiatric evaluations and medication management require 24 hrs notice. 

  3. All cancellations that are done with less than 24 hrs notice will be subject to a cancellation fee.

Insurance Information

  1. Please be advised that Dr Jessica Evelson, MD at Premier Psychiatry of South Florida is not contracted with any insurance panels.

  2. A super bill can be given to the patient for submission to their insurance carrier for possible  reimbursement.

Patient Portal

At Premier Psychiatry of South Florida, patients have access to their patient portal which will allow them to see their upcoming appointments, access their virtual visits, send and receive secure messages to and from Dr Evelson, fill out new patient paperwork and  be able to see statements and securely pay for visits.
